Always test the printed QR — not just the digital file — at the actual viewing distance and lighting. Scanability degrades at small sizes, on glossy paper, and through dirty windows. For posters, aim for a printed QR at least 3.5cm wide; for packaging, at least 2cm. Add a quiet zone (white border) of 4 modules on every side. If you put a logo in the centre, keep it under 25% of the QR area or the error correction will give up.
